Position Summary
The Speech-Language Pathologist (SLP) is responsible for evaluating, diagnosing, and treating patients with speech, language, voice, cognitive-communication, and swallowing disorders in the home setting. The SLP works collaboratively with physicians, nurses, therapists, and caregivers to develop and implement individualized treatment plans that promote optimal patient outcomes, safety, and independence.
Key Responsibilities
- Conduct comprehensive speech, language, cognitive, and swallowing evaluations in patients’ homes
- Develop individualized treatment plans based on physician orders and clinical findings
- Provide skilled speech therapy interventions in accordance with state and federal regulations
- Assess and treat dysphagia, including implementing safe swallowing techniques and diet recommendations
- Educate patients and caregivers on communication strategies, swallowing safety, and home exercise programs
- Document all evaluations, treatment visits, progress notes, and discharge summaries accurately and timely
- Participate in interdisciplinary care planning and case conferences
- Monitor patient progress and modify treatment plans as appropriate
- Ensure compliance with agency policies, Medicare/Medicaid guidelines, and accreditation standards
- Maintain professional licensure and continuing education requirements
Qualifications
- Master’s degree in Speech-Language Pathology from an accredited program
- Current, active state SLP license
- Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA) preferred
- Minimum of one (1) year clinical experience preferred (home health experience a plus)
- Current CPR certification
- Valid driver’s license and reliable transportation
- Strong communication, documentation, and organizational skills
- Ability to work independently in a home-based environment
Physical Requirements
- Ability to travel to patient homes
- Ability to lift, transfer, and assist patients as needed (up to 50 lbs with assistance)
- Ability to stand, bend, kneel, and sit for extended periods
Work Environment
- Patient homes within assigned geographic territory
- Flexible scheduling based on patient needs
- Collaboration with interdisciplinary healthcare team
This is a remote position.
Compensation: $90.00 - $115.00 per hour
